Purposes of the treatment

The purpose of data processing is the following:

• Provision of health or social assistance: guarantee the registration and monitoring of the health and social treatment provided at the centers of the Integral Health Consortium (CSI); ensure the continuity of care between the different care devices; provide the necessary information for the correct invoicing of the cost of the services provided; complete the clinical history in the center, implement the mechanisms of coordination of clinical histories that are developed and the preparation of observational studies with the aim of improving the quality of care.
• Communication of citations via SMS or email: send users of the Integral Health Consortium an SMS or e-mail reminding them of scheduled visits with the centers' services. We will use the data provided directly by the user.
• Teaching: the Consorci Sanitari Integral has a teaching vocation, inherent in the health sector, and at all times the regulations on data protection and specific in this matter will be respected and will conform to the best practices in privacy and information security. This implies that, prior to the start of the care activity, the professional will inform the patient or their representative about the presence and possible action by the students, requesting their consent, which can be verbal. If it is decided not to consent, the students will not be able to be present. In our centers there are students from various universities and professional schools, and graduates in Medicine and Nursing training in different specialties: internal medicine resident (MIR) and internal nurse resident (IIR), according to the accreditation of the Ministry of Health.
• Research: the Consorci Sanitari Integral participates in clinical research projects, subject to the approval of the Clinical Research Ethics Committee of the Biomedical Research Institute of Bellvitge (IDIBELL). To carry out these studies, pseudonymised data will be used, which in no case allow patients to be identified, and consent will be requested beforehand.
• Satisfaction surveys and/or service quality: with the intention of improving the quality of care activity, patients can be selected to consult their experience at the CSI. This survey is voluntary and its result allows the CSI to offer a better service.
• Sending newsletter: sending the periodic newsletter (with subscription) with information of interest from the Integral Health Consortium.
• Video surveillance: guarantee the safety of people, goods and facilities of the Integral Health Consortium. ICU surveillance monitors do not save the image.

Legitimation basis of treatment

The legal bases that protect these treatments are:

• Provision of health or social assistance: treatment necessary to carry out the provision of health and social assistance, based on the consent of the interested party, and the management of health and social assistance systems and services.
• Communication of citations via SMS or email: the same basis that protects the care provision, as communication with the patient is part of their care process.
• Teaching: the consent provided for in the specific regulations (which will be obtained when necessary).
• Research: treatment necessary for scientific research, based on the patient's consent.
• Satisfaction surveys and/or service quality: legitimate interest (sending the survey) and consent (responding to the survey is voluntary).
• Sending newsletter: your consent, which you can withdraw at any time.
• Video surveillance: by virtue of a public interest.

Recipients of the treatment of health data

This information will be used by the administrative services and services directly linked to the health and social care of the Integral Health Consortium, each in their respective areas of competence, and may be sent in whole or in part to public and private official bodies that, for legal reasons or necessity material, they must access the data for the purpose of the correct provision of health and social assistance which constitutes the purpose of the processing of this data.

The data of the patients to whom assistance is provided will be integrated in the Shared Clinical History of Catalonia, in accordance with the legal requirements and following the instructions of the Department of Health of the Generalitat of Catalonia, which is responsible for this file.

If you wish to receive more information or wish to exercise your rights of access, rectification, opposition, deletion, limitation or portability, you can contact the data protection representative of the Consorci Sanitari Integral.

International transfers

Your personal data will not be communicated or processed outside the European Economic Area, except in cases where the patient is a resident of a foreign country and the transfer is necessary for the execution of a contract linked to the provision of the assistance service

Conservation criteria

The data provided will be kept in accordance with health and data protection legislation in force at all times and, subsequently, as long as Consorci Sanitari Integral may have some type of legal responsibility.

Rights of interested persons regarding data processing

The patient/user is responsible for the veracity, updating and correction of the data they provide to the CSI and has the right to obtain information about the processing carried out by the CSl on their personal data. Specifically:

• Right to access your personal data to know the purposes of treatment, as well as request the rectification of inaccurate data or, where appropriate, request the deletion of data that is no longer necessary for the purposes collected.
• Right to request the limitation of data processing. In this case, the entity may retain the data for the exercise of rights or the defense of claims.
• In certain circumstances and for reasons related to your particular situation, you may object to the processing of your data. The CSI will stop processing the data, except for compelling legitimate reasons or in the exercise or defense of possible claims.
• You have the right to portability, that is to say, to the fact that the personal data you have provided are transmitted directly to another person in charge in a structured, commonly used and machine-readable format, when technically possible.

At any time you can withdraw consent for the processing of your data for the purposes indicated when the basis of legitimation is consent. The effects will be from its revocation. You will not be able to withdraw consent for those data treatments that have not required it.

The CSI will respond to requests as soon as possible and, in any case, within one month. If this is not the case, we ask that we apologize and we will do so as soon as possible, given the system of month-to-month extensions of up to two additional months given by the Regulation in case of complexity or volume of requests. However, we remain at your disposal if you wish to contact us again so that we can attend to you and correct any possible technical error that prevented us from responding within this period.

In the event that you have not obtained satisfaction in the exercise of your rights, you can submit a claim to the Catalan Data Protection Authority, or to the corresponding control authority. In any case, before starting any claim, we ask that you contact us by filling out the following contact form of the data protection officer with the purpose of trying to resolve any discrepancy amicably.
