You only need to fill it in with your details if you are making the request on behalf of someone else. In this case, you must also attach a completed copy of the following document (download), a copy of your identity document (DNI, NIE, passport...) and a copy of the identity document of the patient who requires the documentation.

If you have a death of the holder, it is necessary to provide the family book or original document that proves the relationship between the applicant and the holder of the report.

*Important note: without the submission of these documents, the application cannot be processed.
