The 2025 community lunch sessions begin at the CAP of l'Hospitalet del CSI

These are monthly meetings where representatives of community organizations and resources visit the CAPs to present their services.

Last week, with the presentation of Grup Parkinson Club Tennis Taula l'Hospitalet, CAP Collblanc and CAP Torrassa 2025 started a new stage of the community lunch sessions, an initiative launched in 2024 and which has already demonstrated its great impact on improving the well-being of citizens.

The main objective of the project is connect healthcare professionals with the territory's health assets to promote social prescribing. The first steps were taken in 2023 with several strategic actions. From the beginning, the driving group worked to raise awareness healthcare personnel on the importance of social prescribing, an innovative tool that connecta citizens with community resources to address non-clinical needs. Through training sessions, the role of professionals in promoting the community welfare and a detailed map of health assets in the territory was drawn up, a key tool to facilitate access to available resources.

And during 2024, community lunches were consolidated, some monthly meetings where representatives ofcommunity organizations and resources visit the CAPs for present your services. These one-hour sessions have allowed for in-depth knowledge of the resources available, a description of the target population, an explanation of the activities they carry out and a detailed description of the referral processes. Some of the entities that have attended the meetings have been: Fundació Akwaba, Club Social Relacion't, Cor a Cor, Casal Cívic i Comunitàri Progrès, Biblioteca Josep Janés, etc.

To ensure the integration of this information, a systematic practice has been established ofsending summaries information to the entire healthcare team after each session. This has made it easier for professionals to incorporate these resources into their daily practices in an agile and effective way.

The results obtained have been very significant. In just 15 months, the number of social prescriptions has gone from 63 in 2022 to 240 in the first quarter of 2024This increase not only reflects a greater adoption of this practice, but also the positive impact it has on the lives of the people served. In addition, the project has managed to strengthen the community network and improve collaboration between the health system and local resources.
