About the Observatory

The mission of the Gynecology and Obstetrics Service is to provide comprehensive care to the women in its area of ​​health influence during the different stages of life and to be the center of reference in the control of high-risk pregnancies and care at birth

It also deals with complex pathologies, such as dysfunctions of the pelvic floor and its reparative surgery, the comprehensive management of chronic pelvic pain or diagnostic and surgical hysteroscopy. It is made up of multidisciplinary units that collaborate closely with other professionals in the center and with the primary care areas through joint protocols.

The service is organized in units that deal with a specific area of ​​knowledge of pathology, to favor the specialization of the team in complex surgical techniques or in rare diseases and to prioritize the individualized assessment of the patient and thus adapt your care process.

  • Area of ​​Maternal and Fetal Medicine and Area of ​​Neonatology. It includes the entire ambulatory process, delivery and hospitalization of the pregnant woman and the baby in close coordination with the primary care area (the sexual and reproductive health care units, ASSIR) and the third care center reference level (Sant Joan Despí Hospital)

In the care of patients, the service promotes multidisciplinarity, that is to say, that professionals from other specialties participate whenever necessary.
With regard to the academic field, the service has always been committed to training and teaching, both undergraduate students of Medicine at the University of Barcelona, ​​as well as resident internal medicine doctors (MIR) and resident internal midwives (LLIR ).

Medical and surgical emergency area
  • 24-hour continuous assistance with the physical presence of two gynecologists and two midwives
  • Assistance to emergency gynecology and obstetrics consultations
  • Gynecological emergency surgery: laparoscopy, endometrial scraping

Maternal and fetal medicine area
  • Early Pregnancy Control Unit
  • Low-risk gestational visit (welcome visit prior to childbirth assistance coordinated with primary care ASSIRs). Includes fetal well-being monitoring tests at 40 weeks: cardiotocographic recording (RCTG) and biophysical profile
  • High Risk Gestation Unit. Comprehensive medical examination with obstetric ultrasound according to fetal or maternal risk conditions, such as fetal growth disorders, hypertensive states of pregnancy, risk of prematurity, multiple gestation, fetal isoimmunization, perinatal infections or other maternal pathologies
    • Gestational visit of pregnant women with diabetes and other endocrine diseases
    • Infections: Chagas, hepatitis
    • Psychiatry

Prenatal Diagnosis Unit:

  • Morphological anatomical ultrasound (20-22 weeks)
  • Doppler ultrasound of fetal growth (study of maternal-fetal hemodynamics)
  • Aneuploidy screening with an individualized risk assessment
  • Preeclampsia screening with individualized risk calculation
  • Prenatal diagnosis and advice on malformative pathology
  • Invasive prenatal diagnosis techniques (amniocentesis, chorion biopsy with conventional and/or molecular karyotype study)
  • 3/4D ultrasound (defined medical indications)
  • Fetal cardiology
  • Fetal neurosonography
  • Preconception and prenatal genetic counseling

Childbirth and puerperium assistance:

  • Obstetric intermediate care unit (UCOI)
  • Natural childbirth, minimally medicalized childbirth of low complexity and high-risk childbirth
  • Hospitalization of pregnant and postpartum women in individual rooms
  • Breastfeeding Support Program
General gynecology
  • Comprehensive visit where the medical visit and transvaginal and abdominal gynecological ultrasound with Doppler and 3D ultrasound are performed.
  • Diagnosis and medical treatment of benign diseases of the female genital tract.
  • Surgical treatment of benign diseases of the female genital tract:
    • Basic endoscopic surgery (cystectomy, myomectomy, adhesion syndrome, exploratory laparoscopy)
    • Advanced endoscopic surgery: total hysterectomy, endometriosis
    • Classic abdominal and vaginal surgery: hysterectomy, myomectomy, cystectomy, adnexectomy

Gynecological Oncology Functional Unit

Multidisciplinary unit for the treatment of gynecological neoplasms. Surgical, medical and radiotherapy treatment of cancer of the cervix, ovary, endometrium and vulva, coordinated with the Catalan Institute of Oncology (ICO) of Bellvitge Hospital

Functional Unit of Pathology of the Lower Genital Tract
  • Diagnosis and management of cervical and vulvar pathology
  • Diagnosis and management of sexually transmitted diseases in coordination with the infectious diseases service and the ASSIR

  • Surgical treatment of cervical dysplasia, condylomatosis and vulvar lesions
Functional Unit of Pelvic Floor Dysfunctions, Chronic Pelvic Pain and Sexuality
  • Multidisciplinary consultation of the pelvic floor
  • Urodynamics and pelvic floor rehabilitation
  • Specific consultation for chronic pelvic pain in which a comprehensive and multidisciplinary diagnostic and therapeutic approach is taken
  • Vaginal and laparoscopic surgery: vaginal hysterectomy, vaginal plastics, minimally invasive techniques for the correction of urinary incontinence and placement of prostheses and meshes, surgical repair of genital prolapse.
Sterility and Gynecological Endocrinology Unit
  • Diagnosis of primary, secondary sterility and repeated abortions
  • Diagnosis and treatment of gynecological endocrinological diseases
Major and Minor Ambulatory Surgery
  • Outpatient hysteroscopic surgery: endometrial polypectomy, IUD removal

Research

Current research projects in Gynecology:
  • Multicentric and international clinical study to evaluate the safety and efficacy of the Essure® medical product (model ess505) to prevent pregnancy in women who wish to use permanent contraception. Principal investigator: Dr. Federico Heredia
  • Evaluation by means of antimullerian hormone (AMH) of the ovarian reserve after the treatment of endometriomas by means of sclerosis with alcohol or surgery. Pilot study. Multicentric study in collaboration with the Bellvitge University Hospital. Collaborating researcher: Dra. Marta Castellarnau.
  • Axillary lymphadenectomy versus axillary radiotherapy in patients with a positive sentinel node after neoadjuvant therapy. ADARNATO Phase III randomized multicenter study.   Collaborating researcher Dr. Juncal Pineros.
  • Observational study to assess the feasibility of ambulatory vaginal hysterectomy. Collaborating researcher: Dr. Cristina Berdie.
  • Study of factors predicting the occurrence of urinary incontinence after pelvic organ prolapse surgery. Collaborating researcher: Dr. Cristina Berdie.
  • Observational study on the viability of an anesthetic thermogel in surgical hysteroscopy procedures that are carried out in the office or ambulatory surgery facilities. Collaborating researcher: Dr. Margalida Sastre.

 

Current research projects in Obstetrics:
  • Observational, multicenter, prospective, open study to evaluate the safety of the mother and father in the treatment of the threat of preterm birth. Multicentric study in collaboration with the Hospital Clínic de Barcelona. Collaborating researcher: Dr. Federico Heredia

Training of internal medicine residents (MIR):

The Multidisciplinary Teaching Unit (UDM) has accredited one place per year for the MIR training in Gynecology (duration of four years) and two places per year for the IIR training of midwives (duration of two years).

The service trains ten residents of the Family Medicine specialty per year.
