You only need to fill it in with your details if you are making the request on behalf of someone else. In this case, a completed copy of the following document must be attached (download), a copy of your identity document (DNI, NIE) and a copy of the identity document of the patient who requires the documentation.

*Important note: without the submission of these documents, the application cannot be processed.
