Salutec@t: New telematic care model for monitoring people with chronic diseases

The Consorci Sanitari Integral, in collaboration with the Catalan Institute of Health, leads this project which consists of the deployment of an innovative healthcare model, oriented towards chronicity, with a territorial and comprehensive vision (centered on the patient) and integrative (different levels of care).

The aging of the population and the constant growth of the number of chronic patients obliges the health systems to consider new care models in order to be able to maintain excellence, which are sustainable, focused on the patient/citizen and which respond to the new realities of society in the coming years.

Salutec@t wants to launch a new care model for chronic patients based on telemedicine. The project has three pillars: monitoring, empowerment and communication. In order to be able to deploy the model, the Integral Health Consortium and the Catalan Health Institute need to be equipped with a technological platform that allows the implementation of non-face-to-face care services, as well as a new model of relationship between professionals and citizens.

The Salutec@t project proposes telemonitoring through intelligent clinical questionnaires and personalized information (education and support for patients and carers) with the help of gamification.

This project has been co-financed by 50% (610.375 euros) by the European Regional Development Fund of the European Union within the framework of the FEDER Operative Program of Catalonia 2014-2020, through the Public Procurement of Health Innovation program.

The objectives of the new model are:

  • Improve the health outcome and quality of life of the population.
  • Improve the satisfaction of both patients and professionals.
  • Improve system efficiency. Make the visits or actions when they are required and at the level of care that is needed.
  • Improve communication between all actors in the health system, changing the relationship model.
  • Create a chronic care model that serves as a frame of reference that can be extrapolated to the rest of the territories.
