About the Observatory

The mission of the Geriatrics Service is to look after the health of the elderly in its territory. Place special emphasis on:

  • Healthy aging
  • The prevention of disability
  • The minimization of the disease and its impact
  • The promotion of early reintegration into the community
  • The support of patients and their families in situations of advanced illness and at the end of life
  • Care planning, contributing to the identification of PCC-MACA patients and the development of individualized intervention plans in complexity).

Its vision is to attend to patients in fragile situations from a comprehensive and integrated perspective within the different devices of the territory.
The values ​​that guide the Geriatrics Service are:

  • Patient-centered care
  • Shared decision-making
  • Follow-up throughout the life process
  • The prioritization of health problems in a manner congruent with the values ​​and preferences of patients.

The service is made up of a multidisciplinary team.

It has care units based on the needs of patients:

  •     Acute hospitalization at Hospital General de l'Hospitalet (HGH)
  •     Emergency support
  •     Hospitalization for intermediate care at Hospital d'Atenció Intermèdia-HSH
  •     Outpatient care at the external consultations of the comprehensive ambulatory assessment teams (EAIA), Hospital Sociosanitari de l’Hospitalet (HSH), day hospitals (medical and intermediate care)
  •     Residential care.

The service works in coordination with the primary health care of the territory and according to the needs, with attention to patients from the Southern Metropolitan Region (Stroke rehabilitation and palliative care patients with special needs (PADES referrals from LH South )

The key work tool is the comprehensive geriatric assessment (VGI).
