About the Observatory
Health Social Work (TSS) assesses the psychosocial needs of people treated for health reasons in our acute hospitals and the intermediate care hospital.
TSS can provide attention to:
- Conventional hospitalization units
- Hospitalization at home
- External consultations/ambulatory care: pre-surgical consultation, EAIA, ESIC, day hospital, PADES Program, Infectious, others
- Emergencies
The social health study and the social health diagnosis are carried out to establish an individualized intervention plan for each person, family and/or group of patients. The plan is prepared together with the other professionals of the care team (medicine, nursing, other professionals) and, if necessary, with the services of the territory (outpatient clinic, Social Services, residences, others).
It also participates in the promotion and prevention of the health of the population served.
